The Mississippi Delta Health Collaborative (MDHC) works to advance efforts to prevent heart disease and stroke in the Delta with the goal of reducing illness, deaths, and related health disparities.
The MDHC works with local partners to educate the public, change public policies, and promote community actions to increase heart disease and stroke prevention with emphasis on the ABCS among those most at risk for heart disease and stroke in the Mississippi Delta.
- Aspirin: Increase low dose aspirin therapy according to recognized guidelines
A1C: Monitor and control blood glucose (Hemoglobin A1c) - Blood pressure: Prevent and control high blood pressure
- Cholesterol: Prevent and control high LDL-cholesterol
- Smoking: Prevent initiation and increase cessation of smoking, and increase the percentage of population protected by smoke-free air laws or regulations.
Why the ABCS? These four strategies address the major risk factors for cardiovascular disease and can help to prevent heart attacks and strokes — in those who have a history of heart disease and those who want to avoid it.
Clinical Initiatives
The goals of the clinical initiatives are to increase access to care for the treatment of heart disease, stroke, and related risk factors in the 18-county Delta Region and to increase access to treatment and diagnostic screening for the ABCS.
Community Initiatives
The goals of the community initiatives are to create environments that promote heart healthy lifestyle choices and help reduce the risk of heart disease, stroke and related risk factors in the 18-county Delta Region and to build public health infrastructure and health systems with the capacity to implement programs that will effectively detect and reduce risk and manage heart disease and stroke.
