Honor Roll Application

Currently, we are accepting applications from hospital antibiotic stewardship programs (acute care, critical access, long-term acute care). We will expand the program to include skilled nursing facilities in the future.

Applications are accepted on a rolling basis. The Honor Roll is updated with new facilities once every quarter. Once approved, the application will remain valid for a period of one year.

  • All acute care hospitals are encouraged to apply.
  • Individual institutions within a healthcare system will need to apply separately to obtain designation for each facility.
